What to Bring With You

Please bring with you the following items when coming to the hospital:

• Personal Identification
• Emergency Contact Information
• Health Insurance Information
• List of Current Medications including any herbal supplements/vitamins
• Your Advanced Directive for Healthcare – Living Will (if you have one)

You may also want to bring with you:

• Personal toiletries
• Change of underwear, socks
• Sleep wear
• Bathrobe
• Slippers
• Dentures/and or Eyeglass Case
• Books/Magazines

Please DO NOT BRING:

• Valuables such as jewelry, large sums of cash, valuable clothes, credit cards, or other items of personal value.
• Electronic devices such as cellular phones, pagers, two way radios, and laptop computers.  These items can interfere with important electronic medical equipment and cannot be used in the hospital.

If you cannot arrange to have someone take these home for you, please ask one of our staff to arrange to have them put in the hospital safe.
